First-person reports of Major Depressive Disorder reveal that when an individual becomes depressed a profound change or ‘shift’ to one’s conscious experience occurs. The depressed person reports that something fundamental to their experience has been disturbed or shifted; a change associated with the common but elusive claim that when depressed one finds oneself in a ‘different world’ detached from reality and other people. Existing attempts to utilise these phenomenological observations in a psychiatric context are challenged by the fact that this (...) experiential ‘shift’ characteristic of depression appears mysterious and resists analysis in scientific terms. This paper offers a way out of this predicament. The hypothesis proposed is that when an individual becomes depressed, the individual departs from a state of ordinary wakeful consciousness and enters a distinctive global state of consciousness akin to dreaming and the psychedelic state. After unpacking and motivating this hypothesis in the context of research in consciousness science, I outline two of its important implications for the neurobiology of depression and psychedelic psychiatry. The upshot is a promising and conceptually well-motivated hypothesis about depression which is apt for empirical uptake and development. (shrink)

Consumers are increasingly able to easily access health information online about food products. However, consumers have difficulty identifying reliable health information from diverse sources along with information about the coronavirus disease (COVID-19) pandemic because the inundation of information (both true and false) overwhelm consumers. We investigated the usefulness of a checklist confirmation scheme for identifying unreliable COVID-19-related health information. Data were collected from June 30–July 1, 2021. First, we measured 700 participants’ baseline health literacy levels by having them read unreliable (...) health information about the efficacy of green tea intake in preventing COVID-19 based on the results of animal experimentation. Second, participants read an explanation with a five-step flowchart of how to identify reliable health information. Thereafter, we remeasured participants’ health literacy levels. To identify the factors hindering the effect of the confirmation scheme, a logistic regression analysis was performed to calculate odds ratios (ORs) and 95% confidence intervals (CIs). Overall, 77.9% (293/376) of those with low health literacy levels at baseline still had low literacy after the intervention. The factor that hindered the confirmation scheme’s usefulness was benefit perceptions of food ingredients (OR: 0.493; 95% CI: 0.252–0.966). Consumers with higher benefit perceptions of a target product faced more difficulties using the confirmation scheme effectively. Therefore, the most effective strategies involve filtering information at the organizational level rather than the individual level, which should help consumers correctly identify misinformation concerning food and health and promote accurate decision-making. In recent years there has been an explosion of interest in Artificial Intelligence (AI) both in health care and academic philosophy. This has been due mainly to the rise of effective machine learning and deep learning algorithms, together with increases in data collection and processing power, which have made rapid progress in many areas. However, use of this technology has brought with it philosophical issues and practical problems, in particular, epistemic and ethical. In this paper the authors, with backgrounds in (...) philosophy, maternity care practice and clinical research, draw upon and extend a recent framework for shared decision-making (SDM) that identified a duty of care to the client's knowledge as a necessary condition for SDM. This duty entails the responsibility to acknowledge and overcome epistemic defeaters. This framework is applied to the use of AI in maternity care, in particular, the use of machine learning and deep learning technology to attempt to enhance electronic fetal monitoring (EFM). In doing so, various sub-kinds of epistemic defeater, namely, transparent, opaque, underdetermined, and inherited defeaters are taxonomized and discussed. The authors argue that, although effective current or future AI-enhanced EFM may impose an epistemic obligation on the part of clinicians to rely on such systems' predictions or diagnoses as input to SDM, such obligations may be overridden by inherited defeaters, caused by a form of algorithmic bias. The existence of inherited defeaters implies that the duty of care to the client's knowledge extends to any situation in which a clinician (or anyone else) is involved in producing training data for a system that will be used in SDM. Any future AI must be capable of assessing women individually, taking into account a wide range of factors including women's preferences, to provide a holistic range of evidence for clinical decision-making. Shared decision-making involves health professionals and patients/clients working together to achieve true person-centred health care. However, this goal is infrequently realized, and most barriers are unknown. Discussion between philosophers, clinicians, and researchers can assist in confronting the epistemic and moral basis of health care, with benefits to all. The aim of this paper is to describe what shared decision-making is, discuss its necessary conditions, and develop a definition that can be used in practice to support excellence in maternity care. Discussion (...) between the authors, with backgrounds in philosophy, clinical maternity care, health care management, and maternity care research, assisted the team to confront established norms in maternity care and challenge the epistemic and moral basis of decision-making for caesarean section. The team concluded that shared decision-making must start in pregnancy and continue throughout labour and birth, with equality in discourse facilitated by the clinician. Clinicians have a duty of care for the adequacy of women's knowledge, which can only be fulfilled when relevant knowledge is offered freely and when personal beliefs and biases that may impinge on decision-making (defeaters) are disclosed. Informed consent is not shared decision-making. Key barriers include existing cultural norms of “the doctor knows best” and “patient acquiescence” that prevent defeaters being acknowledged and discussed and can lead to legal challenges, overuse of medical intervention and, in some areas, obstetric violence. Shared decision-making in maternity care can thus be defined as an enquiry by clinician and expectant woman aimed at deciding upon a course of care or none, which takes the form of a dialogue within which the clinician fulfils their duty of care to the client's knowledge by making available their complete knowledge (based on all types of evidence) and expertise, including an exposition of any relevant and recognized potential defeaters. Abhijit Sen is one of the prominent narrative writers in Contemporary Bengali literature. He is one who has stood ideologically against the inheritance of colonial model in the Bengali narrative world both in terms of form and content consciousness. Off and on, in his writings, Sen has criticized the trend of writing stories of love & sex only in Bengali fiction and pointed out that so called mainstream literature only depicts the life of growing middleclass and rich people, ignoring the (...) struggle for existence of the common mass, their livelihood. Like Debesh Roy, Abhijit maintains the European model of novel is not useful for depicting the heterogeneity of the reality of life of the larger section of people. He says, like the Third World literature, our narrative is to stand politically and ideologically against neo-colonial attack over the heterogeneous culture of marginalized mass and for this. The present day narrative should be emancipated from the domain of time-space contactless middleclass centric story writing, and it should take the way of re-evaluating & re-constructing of myth, folktales & traditions to represent the present day reality. Depiction of reality, in Sen’s idea is a protest against the traditional way of misrepresenting time & space. When we go through a narrative discourse of Abhijit Sen,we should be conscious about the narrative consciousness of the writer can be termed as post-colonial and post-modern in Indian and Bengalee sense, otherwise, the reader may fail to find the path of entry in the narrative world of Sen, and also will not be able to understand the ideological protest against all kind of hegemony, be it in federal productive relation or in the domain of politics & culture. We want to prepare a critical discourse on Abhijit Sen’s novel ‘Megher Nodi’ from post-colonial and post-modern viewpoint of literary criticism. (shrink)

The article describes the story of painting Nana (1881) by Marcel Suchorowsky known as the most expensive painting sold by a painter in the Russian Empire. But the art piece differs a lot from the general line of the local art market situation, which was defined by special institutions, such as the Imperial Academy of Arts in Saint Petersburg. The main aspects are taken into consideration, such as: critical analysis of the painting, the story of the plot, which refers (...) to Émile Zola’s novel Nana, the painter’s individual and innovative exposition strategy, etc. The history of Nana and its special exhibitions in Europe and the USA through the period of 1981–2012 are described. Reports in the USA periodics serve as a special source of analysis, as long as they allow tracing Nana’s movements and public interest. It was exhibited in the “panorama” style using special lightning and additional objects to imitate the three-dimension space and thus became a popular entertainment. Painting Nana is a very interesting cultural phenomenon as long as it obtained notorious glory due to its provocative plot and an excellent academic technique. Methods: In this paper the design of the proposed Expert System which was produced to help Fruits Agricultural Specialist in diagnosing many of the Pineapple diseases such as : Phytophthora heart (top) rot, Base (butt) rot or Fruit let core rot (green eye, Pineapple Sprain, Turf Toe, Pineapple disease , Plantar Fasciitis, Warts, Bunion, Rheumatoid Arthritis, Gout, Heel Spur, Athlete's Pineapple , The proposed expert system presents an overview about Pineapple diseases are given, the cause of diseases are outlined and the treatment of disease whenever possible is given out. CLIPS Expert System language was used for designing and implementing the proposed expert system. Results: The proposed Pineapple diseases diagnosis expert system was evaluated by Agricultural students and they were satisfied with its performance.
